Levels of Care

Routine Home Care

Hospice care provided to patients in their own home.   Includes  patients in assisted living, group homes and nursing homes.

Respite Care

Short term in-patient care provided to the patient when necessary for the purpose of providing a break in caregiving to the patients' caregiver.  It is only provided occasionally for a maximum of 5 days in a Medicare certified facility.

General In-patient Care

For pain control and symptom management that cannot effectively be managed in the patients home.

Continuous Home Care

Provided during times of crisis at times when the patient requires continuous nursing care to achieve palliation or management of acute medical symptoms.  This intensive care is provided in the patient's home or facility where they live.
